This indicator refers to delayed transfers of care that are attributable to both the NHS and social services. It is calculated as the daily average of no. of bed days delayed each day, per 100,000 18+ population in the year to date.
A delayed transfer of care occurs when a patient is ready for transfer from a hospital bed, but is still occupying such a bed. A patient is ready for transfer when a clinical decision has been made that the patient is ready for transfer AND a multi-disciplinary team decision has been made that the patient is ready for transfer AND the patient is safe to discharge/transfer. This indicator measures the number of delayed days per 100,000 population.
